Queen of Pain brings everything necessary to win a game, which makes her the best mid-hero. Especially her high mobility enables her to gank side lanes and kill enemies. If not countered and played properly, QoP stays strong throughout the game. Here is a QoP build you can use to dominate every game despite being her matchups.
Queen of Pain Item Build:
This build of QoP is considered the best and most preferable. Played properly, you can dominate every game with this build. The core items of QoP are Witchblade, Sange & Kaya, BKB, and Shard make her strong. These items make her tanky and give her high magic damage with attack speed. The Shard gives her built-in silence, allowing her to blink in and silence the enemy. The late game items of QoP are refresher, scythe, and Bloodthorn if needed. You can kill any hero on the map with these items. You can use BKB twice for 10 seconds and 2 times hex and deal much damage in this amount of time.
Queen of Pain Skill Build
If you are up against a ranged hero at mid, it is better to take Scream of Pain at level 1, so you can secure the range creep easily. Against range hero Shadow Strike at level 1 is not a good idea because you need to get close to the hero to cast your spell, which is not worth it if you get damage in return. You can just level up Shadow Strike at levels 2 and 3, and it's better to use your Shadow Strike at level 3 rather than at level 2, as it does not deal much damage and wastes mana. QoP is a strong mid-lane hero. Let's discuss some strong heroes that QoP counters.
Tinker
You can easily destroy tinker in the mid-lane with your Shadow Strike. Tinker's laser won't affect you because you will keep getting healed from your Shadow Strike used on him, and you keep spamming shadow strike on him. You can not only dominate tinker mid of the game but also in late-game since you can silence him when you blink on him after getting Shard. That can cancel his Ulti and make his Rearm to 8 seconds cooldown, and you can easily catch him and kill him with BKB.
Batrider
Batrider is one of the easiest mid-matchups for QoP since bat rider's Firefly becomes useless as she can simply blink out. Cast Shadow Strike on bat rider will slow him down, and it will be hard for him to get even near to QoP, and she won't even need to blink out. Even if the blink is on cooldown and he somehow manages to damage QoP with Firefly, She can just use Sonic Wave, which will push him back.
Ember Spirit
QoP can easily harass ember and zone him out from the lane. She can just keep hitting him, and he can't do much since he is a melee hero who can't easily come near to QoP. Ember can juke your Shadow Strike, but if you fake it, you can easily Shadow Strike on him. You can secure both runes with your blink against ember. It's hard for Ember Spirit to Remnant out if QoP used Shadow Strike on him because his Remnant will run slowly, and QoP can easily kill you.

Puck
Puck is a versatile hero with cool spells to counter heroes like Queen of Pain. Puck's Phase Shift allows her to avoid Shadow Strike, Scream of Pain, Sonic Wave, and even hits. Waning Rift allows Puck to silence QoP preventing her from using any spells. If Puck uses Dream Coil on QoP, she cannot blink out. You can use BkB to blink out from the Dream Coil, but if Puck is level 25 and she took the Dream coil pierces Magic immunity, you cannot blink out even if you use BKB. To prevent this, You can get an Aeon disk and simply walk out of Dream Coil or buy a wind waker to escape Dream Coil.
Anti-mage
Anti-mage Counterspell is the best way to reflect Shadow Strike, and Counterspell also gives magic resistance allowing him to take less damage from QoP except for Sonic Wave, which deals Pure Damage. He can easily catch up to you since he has a blink too. Against Anti-Mage Linkens on QoP is a must because AM can easily kill her with Abyssal, Manta, and Mana Void afterward. If you want to cast Shadow Strike on him, you can also do the fake casting technique that applies against Puck.
Meepo
Meepo is a melee hero, but you can't really harass and zone him out from the lane because of two Meepo, so even if you harass one Meepo, he can send that Meepo to base to heal and use the other Meepo. His Earthbind can keep QoP from blinking out or running away. He can use 4 Earthbinds because of his clones. If you get in Earthbind, you are probably dead. BKB is a must to play against Meepo.

Axe
He can catch multiple enemies with his Berserker Call, and you can easily blink in and use Sonic Wave on all of them. Axe can do frontline and can jump in first to allow you to see if you should blink in or not.
Magnus
He can also catch multiple heroes with his RP and drag back an enemy hero to you with his skewer. If he RP on various heroes, you can easily blink in and clean up with your Sonic Wave.
Spirit Breaker
If you have a bad matchup, you can ask Spirit Breaker to charge at mid and help you kill the enemy. Since QoP has such high mobility, QoP and Spirit Breaker can play on the whole map together.
